  • Health history — relevant medical conditions, injuries, medications, allergies, and pregnancy status
  • Session notes — treatment details, observations, and progress

  • To assess suitability for treatment and ensure your safety
  • To plan, deliver, and document your sessions
  • To process payments and issue receipts
  • To communicate appointment reminders or follow-ups
  • To meet regulatory and legal record-keeping obligations

Consent

  • By completing the intake form, you consent to the collection, use, and storage of your personal information for the purposes listed above.
  • You may withdraw consent at any time in writing, understanding that this may limit or prevent our ability to provide services.
  • We will not use your information for marketing without your separate, explicit consent.

How We Protect It

  • Electronic records are password-protected and stored on encrypted devices.
  • Only authorized personnel have access to client files.

Retention

Client records are retained for a minimum of 2 years after your last visit. After the retention period, records are securely destroyed.

Your Rights

Under British Columbia’s Personal Information Protection Act (PIPA), you have the right to:

  • Request access to your personal information on file
  • Request correction of inaccurate information
  • File a complaint with the Office of the Information and Privacy Commissioner for BC (OIPC) if you believe your privacy rights have been violated
